These guidelines, while effective at a population level, have always grappled with balancing the benefits of early detection against potential harms such as overdiagnosis, overtreatment, false positives, and the psychological burden associated with follow-up procedures.

Current guidelines from various prominent organizations illustrate the existing disparities and the lack of consensus. For instance, the ACS recommends annual mammograms for women aged 40-44 if they choose, and annually for women 45-54, then every two years for women 55 and older. In contrast, the U.S. Preventive Services Task Force (USPSTF) recommends biennial screening for women aged 40-74. The American College of Obstetricians and Gynecologists (ACOG) suggests annual or biennial screening for women aged 40-74. These variations underscore the complexity of universal recommendations and highlight the inherent limitations of relying solely on age as a risk factor, especially when research has consistently demonstrated that breast cancer risk can vary dramatically from one individual to another, influenced by a myriad of genetic, biological, and lifestyle factors.

In 2023, an estimated 297,790 new cases of invasive breast cancer and 55,720 cases of non-invasive (in situ) breast cancer were expected to be diagnosed in women in the U.S., with approximately 43,170 women projected to die from the disease. Expanding Genetic Testing: Uncovering Hidden Risks

Since its inception in 2016, the WISDOM study has expanded its reach, enrolling over 80,000 women to date. A more recent phase has included women as young as 30, driven by the critical need to identify those who may face an increased risk of aggressive early-onset cancers due to inherited genetic variants.

One of the study’s most profound and potentially practice-changing findings revolved around genetic testing. A remarkable 30% of women who tested positive for a genetic variant strongly linked to a higher breast cancer risk reported absolutely no family history of the disease. This discovery directly challenges current clinical guidelines, which often require a strong family history of breast or ovarian cancer as a prerequisite for genetic testing referral. Under existing protocols, many of these women with significant genetic predispositions would remain unidentified, delaying crucial interventions.

Beyond well-known pathogenic variants like BRCA1 and BRCA2, the WISDOM study also innovatively evaluated smaller DNA changes that can be aggregated into a polygenic risk score (PRS). A PRS represents an individual’s cumulative genetic predisposition to a disease, calculated by summing the effects of many common genetic variants, each contributing a small amount to overall risk. This sophisticated approach significantly improved the precision of individual risk predictions, leading to 12% to 14% of participants being reassigned to a different risk category based on this more granular genetic data.
