All we know right now is that people who smoke a lot of marijuana—to the point that they develop a dependency on it or require hospital care or evaluation for it—do appear to have a higher cancer risk."

While occasional use might be associated with minimal or negligible risk, chronic, high-volume consumption appears to be a significant factor in elevating cancer risk, drawing parallels to the well-documented effects of heavy tobacco use.

"With tobacco, we do see an increased prevalence of bladder cancer," Dr. Udelsman notes. "Whether that relationship is also the same for heavy marijuana smoking, we don’t know. That’s why we’re trying to determine what the risks are so that people can know what risks they’re taking."

The potential for marijuana to influence bladder cancer risk is of particular interest, given the known carcinogens present in both tobacco and marijuana smoke that are eliminated through the urinary system. Understanding if the same chemical pathways and inflammatory responses are triggered by heavy cannabis use could provide further clarity.

"It’s almost unheard of to get small cell lung cancer without some type of inhalational injury," he observes, suggesting that any form of chronic lung irritation from inhaled substances could contribute to its development.

Non-small cell lung cancer (NSCLC) also appears more frequently among individuals who smoke marijuana. "We also see an increase in non-small cell lung cancer in smokers of both tobacco and marijuana, so both types are increased when compared to nonsmokers," he adds, highlighting that the risks appear to be compounded when both substances are consumed.

The Emerging Landscape of Vaping

The advent of electronic cigarettes and vaping has introduced another layer of complexity to the discussion of inhaled substances and health. Initially perceived as a safer alternative to traditional smoking, vaping has increasingly been linked to serious inflammatory lung diseases, though direct causal links to cancer are still under investigation.

"But we are starting to see some very severe inflammatory diseases—not cancers, but benign diseases—from vaping," Dr. Udelsman reports. The long-term effects of vaping, particularly its potential to induce cancer, are difficult to assess due to its relatively recent widespread adoption. Researchers have only had about 15 years of common usage, a timeframe insufficient for definitive conclusions on cancer risk.

This uncertainty extends to vaping marijuana. "The data on vaping is very new, so we don’t know yet, but I’d worry about anything you’re breathing into your lungs because it infiltrates the cells and air sacs in your lungs, which can cause damage and put you at a higher risk for cancer," Dr. Udelsman cautions. The concern is that the fine particles and chemical constituents in vape aerosols, regardless of their origin, could lead to cellular damage and inflammation over time.

Occasional Use vs. Chronic Dependency: A Critical Distinction

Despite the emerging evidence of potential cancer risks associated with marijuana use, Dr. Udelsman maintains a nuanced perspective, differentiating between occasional recreational use and chronic, heavy consumption. He does not anticipate that occasional recreational marijuana use will trigger a widespread lung cancer epidemic.

"I worry less about occasional use and the slight inflammation that it might cause for a temporary period. Your body probably recovers from that very quickly, with very little damage," he states. "I’m more worried about chronic, heavy exposure: people who are using marijuana every day, multiple times a day. That’s where I think you can start to build up injury. Those are the people data today shows are at risk for developing cancer, similar to tobacco."
