Researchers at the Botucatu School of Medicine at São Paulo State University (FMB-UNESP) in Brazil have unveiled compelling preliminary findings suggesting that a readily available vitamin D supplement could significantly enhance the efficacy of chemotherapy for women diagnosed with breast cancer. This groundbreaking study, funded by the São Paulo Research Foundation (FAPESP), indicates that low doses of vitamin D may not only improve treatment outcomes but also offer a more accessible and cost-effective alternative compared to some of the more expensive or difficult-to-obtain drugs currently employed to boost chemotherapy response. The clinical trial, conducted at the oncology outpatient clinic of the general and teaching hospital at FMB-UNESP, involved 80 women aged 45 and older who were on the cusp of commencing neoadjuvant chemotherapy. Neoadjuvant chemotherapy, a critical component of breast cancer treatment, is administered prior to surgical intervention with the primary goals of shrinking tumors, thereby facilitating easier surgical removal, and potentially targeting microscopic cancer cells that may have spread beyond the primary tumor site.

One cohort received a daily oral supplement of 2,000 International Units (IU) of vitamin D, a dosage considered moderate and well within generally accepted safe limits. Current public health guidelines for vitamin D intake vary by age and specific health recommendations. Generally, for most adults, a daily intake of 600 IU is recommended, while individuals aged 70 and older are advised to consume 800 IU daily to account for age-related declines in skin synthesis efficiency and absorption. The American Academy of Pediatrics recommends 400 IU daily for infants. It is important to note that while vitamin D is essential, excessive intake can be detrimental. Hypervitaminosis D, a condition resulting from dangerously high blood levels of vitamin D, can lead to a range of adverse effects, including nausea, vomiting, weakness, bone pain, and the formation of kidney stones due to excessive calcium absorption.
Prevalent Vitamin D Deficiency and its Link to Treatment Outcomes
A significant observation at the commencement of the FMB-UNESP study was the widespread prevalence of vitamin D deficiency among the participants. The majority of the 80 women exhibited low vitamin D levels, defined by the researchers as less than 20 nanograms per milliliter (ng/mL) of blood. This threshold is considerably lower than the optimal levels recommended by health authorities. "With supplementation, levels increased throughout chemotherapy treatment, which reinforces a possible contribution to the patients’ recovery," he told Agência FAPESP. He further emphasized the practical advantages of vitamin D supplementation. "Vitamin D is an accessible and inexpensive option compared to other drugs used to improve the response to chemotherapy, some of which are not even included in the list of the Unified Health System [the Brazilian national public health network, known as the SUS, its acronym in Portuguese]." This assertion points to a potential paradigm shift, where a fundamental nutrient could serve as a vital adjunct in cancer care, democratizing access to enhanced treatment efficacy.

"These are encouraging results that justify a new round of studies with a larger number of participants," Carvalho-Pessoa concluded. "This will allow a greater understanding of the role of vitamin D in increasing the response to chemotherapy treatment and, consequently, in the greater likelihood of breast cancer remission."
