This sustained smoking from a young age significantly elevates the risk of premature heart damage, a finding that underscores the profound and early impact of nicotine on cardiovascular health. The research, a collaborative effort between the Universities of Bristol and Exeter in the United Kingdom, and the University of Eastern Finland, has been published in the prestigious Journal of the American College of Cardiology (JACC), a leading publication in cardiovascular research.
The Pervasive Grip of Childhood Smoking
The study meticulously followed a cohort of 1,931 children from the University of Bristol’s renowned Children of the 90s study, tracking their smoking habits and cardiovascular health from the age of 10 years until they reached 24 years of age. Even after meticulously accounting for a comprehensive array of other cardiovascular risk factors – such as elevated blood pressure, obesity, systemic inflammation, abnormal lipid profiles (dyslipidemia), and sedentary lifestyles – the direct detrimental effect of tobacco smoking on heart structure and function remained significant. The study found that the direct impact of tobacco smoking on increased heart size during the critical growth period from age 17 through 24 years was still a substantial 30%. This finding is particularly important as it isolates the impact of smoking itself, separate from other lifestyle and biological factors that often co-exist with smoking.

While previous research has established a strong association between adolescent smoking and an increased risk of cardiovascular death in later life, typically in the mid-fifties, this new study breaks new ground by examining the earliest observable manifestations of long-term tobacco smoking on the heart from childhood. Prior to this research, the scientific community lacked concrete evidence on how sustained smoking from such a young age affected the developing heart. The scarcity of such data is attributed to the significant logistical and ethical challenges involved in conducting repeated echocardiography (ultrasound of the heart) assessments in large populations of healthy young individuals over extended periods. This study’s groundbreaking longitudinal design and extensive data collection have filled this critical knowledge gap.
A Rigorous and Comprehensive Study Design
The current study stands as the largest and longest-duration investigation worldwide to combine detailed assessments of active tobacco smoking with repeated echocardiographic measurements of heart structure and function in a young cohort. The participants provided crucial data through questionnaires administered at multiple intervals: ages 10, 13, 15, 17, and 24 years, meticulously documenting their tobacco smoking habits.
To assess cardiovascular health, echocardiography measurements of the heart’s structure and function were conducted at two key developmental stages: ages 17 and 24 years. This allowed researchers to track changes over time. Complementing these imaging assessments, participants’ fasting blood samples were repeatedly analyzed to measure a range of biomarkers critical for cardiovascular health. These included levels of low-density lipoprotein cholesterol (LDL-C, often referred to as "bad" cholesterol), high-density lipoprotein cholesterol (HDL-C, the "good" cholesterol), triglycerides, glucose, insulin, and high-sensitivity C-reactive protein (hs-CRP), a marker of inflammation.
The research team also collected a wealth of other data to ensure a comprehensive analysis. This included regular measurements of blood pressure and heart rate, information on socio-economic status, family history of cardiovascular disease, accelerometer-measured sedentary behavior and physical activity levels, and dual-energy X-ray absorptiometry (DXA) scans to determine fat mass and lean mass. The statistical models employed were sophisticated enough to account for all these potential confounding factors, thereby isolating the direct impact of tobacco smoking.
The Emerging Threat of Vaping and E-cigarettes
Dr. Andrew Agbaje, an award-winning physician and Associate Professor (Docent) of Clinical Epidemiology and Child Health at the University of Eastern Finland, a lead researcher on the study, emphasized the broader implications of these findings in the context of current youth substance use trends. "This current study could be extrapolated to vaping and e-cigarette users who might unknowingly be at risk of significant and irreversible heart damage," he warned. Let us say NO to tobacco and its fancy products in order to save the lives and future health of our children and adolescents."
