A groundbreaking study conducted by researchers at MedUni Vienna and University Hospital Vienna has unveiled the significant potential of a novel viral therapy, Talimogene Laherparepvec (TVEC), in treating locally advanced basal cell carcinomas (BCCs), the most prevalent form of skin cancer. The findings, published in the esteemed journal Nature Cancer, demonstrate that TVEC not only effectively reduces the size of these stubborn tumors, thereby simplifying surgical removal, but also induces complete regression in a notable proportion of patients, offering a less invasive and potentially disfiguring treatment alternative.
The Challenge of Locally Advanced Basal Cell Carcinoma
Basal cell carcinoma, while typically slow-growing and rarely metastasizing, can present significant challenges when it reaches a locally advanced stage. These tumors often develop in chronically sun-exposed areas, predominantly the face, and can infiltrate surrounding tissues, including cartilage, bone, and nerves. TVEC: A Dual-Action Viral Agent
The focus of the MedUni Vienna study was Talimogene Laherparepvec (TVEC), a genetically engineered oncolytic virus derived from the herpes simplex virus type 1. While TVEC has previously received regulatory approval for the treatment of unresectable melanoma metastases, its application in BCCs represented a novel frontier. Oncolytic viruses like TVEC are designed to selectively infect and replicate within cancer cells, leading to their lysis (bursting) and release of tumor-associated antigens. Crucially, this process simultaneously triggers an immune response against the tumor. By presenting these antigens to the immune system, TVEC aims to elicit a systemic anti-tumor immunity, which can help the body recognize and eliminate cancer cells not directly targeted by the virus. This dual mechanism of direct tumor cell destruction and immune system activation positions TVEC as a potent therapeutic agent.
The Study Design and Patient Cohort
The clinical trial involved 18 patients diagnosed with locally advanced basal cell carcinomas. These individuals were candidates for extensive surgical interventions, such as flap or skin graft procedures, due to the size and anatomical location of their tumors. The primary objective of the study was to administer TVEC as a neoadjuvant therapy to shrink these tumors, thereby facilitating surgical removal with less morbidity and improved cosmetic outcomes.
Over a period of 13 weeks, each participant received six intralesional injections of TVEC directly into the tumor site. Following this treatment regimen, the tumors were surgically excised. The researchers meticulously monitored tumor response, both clinically and histologically, and assessed the safety and tolerability of the treatment.
Promising Results: Tumor Shrinkage and Complete Regression

"This enabled the tumour to be reduced in size in half of the patients to such an extent that surgery with direct wound closure was possible," stated Dr. Höller, highlighting the profound impact on surgical complexity. He further elaborated on the remarkable therapeutic effect: "In a third of the cases, the subsequent histological examination even showed no more living tumour cells." This indicates complete tumor eradication in a substantial subset of patients, a feat rarely achieved with solely neoadjuvant therapies for such aggressive BCCs.
Crucially, no tumor progression was observed during the TVEC treatment period. "All treated tumours at least became smaller, and no tumour grew further under the therapy," Dr. Höller emphasized. The treatment was also found to be well-tolerated, with no severe adverse events reported, a critical factor for any new therapeutic modality.
Julia Ressler, the first author of the study and also from the Department of Dermatology, underscored the clinical implications of these findings. "The new treatment option for basal cell carcinoma can not only simplify surgery, but also help to avoid disfiguring operations and functional limitations," she remarked, emphasizing the significant improvement in patient quality of life.
Immunological Insights: Boosting the Body’s Defense
Beyond the clinical observations, the research team, in collaboration with the St. Anna Children’s Hospital, conducted extensive analyses to elucidate the immunological mechanisms underpinning TVEC’s efficacy. These comprehensive investigations revealed a significant enhancement of the immune defense within the tumor tissue following TVEC therapy. This finding aligns with the known modus operandi of oncolytic viruses, reinforcing the concept that TVEC not only directly attacks cancer cells but also primes the patient’s own immune system to recognize and eliminate the malignancy. Expanding the trial to include a greater number of patients will be crucial for confirming the reproducibility of these results and for further assessing the long-term efficacy and safety profile of TVEC in this indication.
Moreover, the researchers are likely exploring the potential of TVEC in other types of skin cancers or even in combination with other treatment modalities, such as immunotherapy or radiation therapy, to achieve synergistic effects. The detailed immunological data generated in this study will also be invaluable for guiding future therapeutic development and patient selection.
